Bonnie,
We created a paper screening tool for our RRT that is based on assessing for 
SIRS and Organ dysfunction. It uses a check box system to assess for Sepsis, 
Severe Sepsis, or Septic Shock and our department uses it to collect data.  If 
certain boxes are checked indicating the severity, then there is indication to 
use a sepsis protocol. 
I also created a modified version for the non ICU areas to be able to compile 
objective information to communicate to the RTT/physician. At the bottom of 
that tool it states that if the patient meets the above criteria call the RRT.

Collection and analysis of data from bundle compliance is crucial to the 
success of improvement efforts in the care of patients with severe sepsis and 
septic shock. The Surviving Sepsis Campaign's (SSC) electronic data collection 
tool has been updated to reflect the 2012 guidelines and bundles and allows 
institutions to measure activities as improvement cycles are implemented. 
Additionally, a new step-wise approach has been developed, which will assist 
institutions new to the bundles.
Learn to use the new data collection tool efficiently and effectively in your 
institution with instruction from the primary designer and member of the SSC 
leadership, Christa Schorr, RN, MSN.  Using screen shots of the new resource, 
Ms. Schorr will explain how it differs from the previous database, illustrate 
how users can download and install it locally, and demonstrate how 
participating facilities can generate reports. A question and answer session 
will follow her presentation. This webcast will be held Monday, October 21,

Hospitals worldwide are encouraged to download the new electronic data 
collection tool that is consistent with the 2012 SSC guidelines released in 
February 2013 to improve the care of sepsis patients worldwide. Collecting data 
helps improve outcomes and inform benchmarking data. Hospitals seeking to
