I would also add that the Levy definition used much softer language in identifying sepsis and the current guidance is no longer “2 of 4 SIRS criteria”. The specific language is “Infection, suspected or confirmed, and some of the following” with multiple assessment categories including perfusion, hemodynamic, organ dysfunction, and inflammatory variables. There is no guidance beyond that. While the lack of concrete criteria can frustrate some, I think this is an important distinction emphasizing, as the authors state, that there is no diagnostic gold standard to compare to and that a global assessment of the patient relative to the acute infection is necessary.
Specifically, I am not sure as to why the change was made from 38 to 38.3, there was no footnote provided referencing data supporting the change. Operationally, it would seem a threshold of 38 as one of the screening criteria for more careful review would not be inappropriate given the spirit of the changes to the 2001 criteria.
