I also do the same as Peggy for my hospital. I try to fill out a scorecard
for each Septic Shock case we have. It merely reviews the case from triage to
1st-2nd day in ICU. If we make our time frame for a certain goal then it's
highlighted Green, if we are outside our goal then it's marked Red. Ex: Triage
to Abx : 200 mins I send the scorecard to the ER MD, ER RN, ER educators,
Admitting MD, ICU RN and ICU educators. We have had a lot of great success by
starting this process. I try to send the scorecards within 1-2 days from time
of admission because this helps everyone remember the case. Then I record the
times in an excel spreadsheet to review at our monthly meetings. Attached above
is our scorecard.

I collect (for close to 1-1/2 years) compliance to the 3 hr bundle (each
element individually) by physician/PA/ARNP in the ED for all severe sepsis &
septic shock patients coming through the ED with the diagnosis present on
admission. Compliance measures are done through manual chart abstraction by
me....laborious but proved interesting and quite helpful. Each doc gets his
own score then all providers together get a composite score kept on a heat map
displayed at the monthly ED meeting.
