We have something very similar.  A sepsis alert brings our intensivist to the 
bedside for evaluation.  It also brings a phlebotomist and respiratory 
therapist to the bedside to ensure labs and treatments are completed in a 
timely manner.  The ED RN continues to care for the patient in the ED and 
initiates 3hr bundle elements, but transfer to the ICU is expedited and patient 
is handed off with bedside report.  6 hour elements are almost exclusively 
performed in the ICU.  Our alert is triggered when patient meets: infection + 
SIRS and 1) any hypotension; 2) lactic acid ≥4; and/or 3) 2 or more new organ 
dysfunction.

We have a protocol that the ED initiates an ICU/Sepsis Alert.  A team (critical 
care physician assistant and an ICU RN)  goes down to the ED to assume care of 
the patient, notifies the intensivists,  and moves the patient to an ICU bed 
when available.

We are developing a protocol for all of our severe sepsis patients with either 
hypotension or lactate >4  to go expeditiously to the ICU for initial 
resuscitation...similar to the way we get acute MIs on their way to the cath 
lab in a hurry.   We are trying to reduce ED length of stay and urgent 
transfers to ICU within several hours of admission to non-ICU units.

Does anyone have a similar protocol that you could share?  or guidelines 
related to sepsis ICU admission criteria?
