I think you have asked the million dollar question. The SSC is doing a 
collaborative right now with Med/Surg floors and with PCU's. I think through 
this work we will be able to show that patients will benefit from the same 
fluid bolus when they have a lactate <4. As we move forward with this work we 
will be giving the patients fluid on the Med/Surg floor as well. We are working 
on a Geriatrics floor and we will beginning  with 1.5ml/kg for lactates <4.
We are set to go live with this October 20th.

Hi Everyone,
Our hospital is in the process of going live with an ED Sepsis Alert. We were 
able to get the lab to recognize a critical value for lactate that would 
trigger the lab to call the RN and the RN to notify the provider. My concern is 
that the level we chose was 4 or >. The data that we have collected on our own 
patients so far, even those in septic shock, rarely have a lactate as high as 
4. What are other hospitals using for a lactate to trigger an alert?
