Ellen,
When a patient is "flagged" septic shock in our ED, a rapid response or ICU nurse goes to the ED to assist with the 3 hour bundle. We like to transport these patient ASAP to our ICU or step down unit, so 6 hour bundle is completed outside of the ED. This has improved our 3 hour bundle times to just over an hour and even under 30 minutes. A page is initiated that is identified on beepers carried by the ICU charge nurse. When they receive the page, they initiate a call to the ED and if requested by the ED, will respond. They assist with the central line insertion, and CVP/SVO2. The biggest challenge with this is if the charge nurses are occupied with patient care, there is a delay in response. Another opportunity that we found is that the ED would manage without Paging it out, causing fall outs on completion of the 6 hour bundle. We're stalled a little right now, and I'm contemplating sending an ICU nurse as a responder when we have an identified septic shock patient in the ED. Does anyone do this? How is it operationalized? Has it been successful? If this has been tried and failed, why did it fail? thanks Ellen Ellen M Donnelly Nurse Manager 6N MICU and Intermediate Care Unit St Joseph Mercy Hospital Ann Arbor tel: 734-712-6654 fax: 734-712-6656 email: [email protected]<mailto:[email protected]> www.stjoeshealth.org<http://www.stjoeshealth.org/> Facebook<http://www.facebook.com/sjmhs> / Twitter<http://www.twitter.com/stjoes_health> [cid:[email protected]] Confidentiality Notice: This e-mail, including any attachments is the property of Trinity Health and is intended for the sole use of the intended recipient(s).
