Subject: [Sepsis Groups] Septic shock presentation

I have come across a chart that the MD documentation notes sepsis with shock 
but the patient does not meet criteria for septic shock.  The patient is 
positive for severe sepsis but does not have a lactic acid ≥4 or hypotension.  
Do I still note this patient as septic shock due to MD documentation?
