Hello,

We currently have a Resident team doing a QI project for sepsis. I told them 
about the reluctance to give the full 30 ml/kg bolus to any patient regardless 
of ESRD or CHF.

We met yesterday and they had some suggestions for our order sets in regards to 
fluids. I also proposed that they add an area to the order set for the nurse to 
perform Positive Pulse Pressures at certain intervals while receiving fluids. 
They are presenting at our Sepsis Collaborative next week and presenting at KU 
School of Medicine the day after. I am certain that there will be much 
discussion on this at the collaborative.

I will provide more detail late next week and their initial study. If we adopt 
their suggestions, we will keep data for them over the next year in regards to 
this. They are hoping to eventually publish the study.

I am hoping that the QI project at Wesley Healthcare will benefit us all!

What have been your strategies for persuading physicians to continue with fluid 
resuscitation in the patients who are positive for severe sepsis but also have 
ESRD or CHF.  Any specific studies would be helpful as well.
