Please advise on the following scenario regarding presentation day. The patient was admitted without meeting severe sepsis criteria. On day 6 of admission the MD documents "severe sepsis". Five days later, hospital day 11, the MD documents "sepsis on admission with septic shock". Which day is presentation day? Day 6 when MD wrote "severe sepsis" OR day 11 when MD wrote "septic shock on admission" which would actually mean presentation day was day of admission?
