If documentation of a positive urine or blood culture is available PRIOR to physician or RN documentation of known or suspected infection, can the time of the positive culture be used as the time of known or suspected infection?
I have a patient with a positive urine culture (taken from a straight cath in the ED) but no mention of UTI by the ED physician or the admitting physician. UTI with Severe Sepsis is given as a discharge diagnosis.
