The requirements for the 30  ml/kg bolus have not changed and are either 
hypotension  or Lactate > 4.0 in the face of sepsis
Some clinicians choose to give this amount for a lactate >2 to avoid 
progression to shock.

Subject: [External] [Sepsis Groups] Lactate for fluid

Our institution seems to have some confusion in regards to what the lactate 
cutoff is for the 30ml/kg of fluid resucitation in the 3 hr bundle. Are we 
supposed to be using 2 or 4 as the cutoff per CMS?  I know Surviving Sepsis 
uses 4.
