To clarify, now no actual assessment needs to be documented, just that the reassessment was complete? Does it need to be signed off by a nurse or physician?

I have reviewed or performed all elements of the Septic Shock focused exam; or two of the four bedside hemodynamic assessment elements." According to a Quality Measures Reporting Webinar on 1/11/17 titled "SEP-1 Early Management Bundle, Severe Sepsis/Septic Shock: v5.2 Measure Updates" (found under IQR archived events) even a simple phrase such as "Sepsis focus exam completed" will meet this portion of SEP-1.
