Please find attached an example copy of the Individual Clinician Feedback 
report that we provide to all clinicians involved in patients who require 
admission to critical care as a result of sepsis.

These have been in use at Nottingham University Hospitals as a form of feedback 
since 2012 with great success. As a result of this, they have also been used 
for other "time-critical delivery" acute work streams such as Emergency 
laparotomy and Early Warning Scores at Nottingham University Hospitals NHS 
Trust.

Hello,
I am in search of assistance.   Can somebody explain why 30ml/kg of crystalloid 
fluids are to be given for an initial hypotension.....even if in severe sepsis 
and not in septic shock.   I have many doctors questioning this....They say 
these fluids of 30ml/kg are only to be given if in septic shock.  When we do 
our chart abstractions and answer the question "yes" for initial hypotension, 
it asks if fluids of 30ml/kg were given.  If we say these fluids were not given 
at 30ml/kg, then it falls out.   I have even seen it happen with the lactate <2.

Anybody else having same problems?

You or your staff recently provided PERFECT CARE  for a patient admitted with 
sepsis.  This patient met criteria for Sepsis Care because they had a lactate ? 
4 or SBP <90 despite a 30ml/kg bolus.  Attached is a summary of the performance 
measures for this patient as defined by _________Health System Sepsis 
Committee.  Your continued commitment to quality care and improved patient 
outcomes are truly appreciated.  Thank you!
