What we have done here at CRMC is we have a field in the Nsg documentation 
where the nurse clicks if the patient is positive for severe sepsis or septic 
shock.  If either of those fields are checked then I get a report of the 
discharge phone call where we ask them questions about their stay.  I get about 
8-10 pts per month in my report which is not nearly the 50-75 severe 
sepsis/septic shock pts I have each month but it is a sampling.

Another suggest Joint commission gave me was to bring in a patient after they 
are better and just ask them- what could we have done better? And how was your 
care?  Etc.
This would be more time intensive.
