Who is conducting your sepsis patient phone calls? RN's, other? I am curious as to the types of responses and situations you are experiencing with the sepsis population. Might you have a template you use on the process for these calls. In addition, do you contact the SNF facilities you send these patients to for follow-up? Thank you! If either of those fields are checked then I get a report of the discharge phone call where we ask them questions about their stay. I get about 8-10 pts per month in my report which is not nearly the 50-75 severe sepsis/septic shock pts I have each month but it is a sampling. Another suggest Joint commission gave me was to bring in a patient after they are better and just ask them- what could we have done better? And how was your care? Etc. This would be more time intensive. My question is, are we supposed to use branching logic for abstraction? For example, if we answer 'Yes' to Severe sepsis, but we don't have an Initial Lactate collected and we say 'No' to Initial Lactate question; do we stop at that point or do we have to still go ahead and answer the rest of the questions? If anyone has been validated, what did you do? If you used branching logic (did not answer rest of the questions) was it taken against the hospital and considered as 'wrong' abstraction?
