Hi Mary - we are working with our SNFs also. Check out the HSAG site. There are 2 webinars and a handout etc. you can find by going to the calendar and click on the sepsis event. The site is open to all.
We met with our local SNFs and HH a few weeks ago to plan education about sepsis and heart failure for early recognition and management. We are going to start with showing staff the HSAG webinars - one also includes SBAR for sepsis. Hope this helps. Good luck!
