I had come across an excellent tool (picture based - resident behavior changes) 
for screening for sepsis / recognize early changes in a long-term care setting. 
 I'm having trouble locating it again.

Does this sound familiar to anyone? If so, is anyone able to point me in the 
right direction or send a copy?
(I found another site that had similar tools, but the one I saw before was very 
simplistic, excellent and able to relate to almost any education level.)
