So I fail the bundle compliance if there is no documentation found at 3 hr and 6 hr. Even though all other components are present. Most times the patient is leaving the ER before or at 3 hours and by the time the Attending if admitted sees it is outside of 3 hr parameter. I was wondering if anyone else counts the entire bundle compliance as fail if the documentation is not there at the accurate parameter for Focused re-exam. It's frustrating because we don't get credit for what is being done for Bundle compliance for these patients. Would appreciate other observations.
