RNs are screening at SFH when triggered by the presence of 2 SIRS. The EMR
begins screening and sends an alert when 2 SIRS criteria met in the EMR. The
pop-up alert requires the RN to answer whether the patient has a suspected
infecti0n.

If yes to infection, a severe sepsis screening flowsheet automatically opens.

This is instituted hospital-wide and screening takes place whether in Triage,
Main ED, or in-patient areas if 2 SIRS plus infection are present.

We screen all patients in ED triage using a paper form at this point. The
nurses follow a protocol to initiate the labs/BCs if positive.

Subject: [Sepsis Groups] Sepsis Screening

I'm hoping to get an idea of what other facilities are doing about ED Sepsis
Screening:

1. Are you screening every patient?

2. Are they screened in Triage or not until they are in a room?

3. If not screening every patient, what are the triggers for screening?

4. Is the screen on paper or electronic?

Thank you in advance for being willing to share your expertise and experiences!

Best,
Claudia
