User specifies controllers list "c1, c2, c3"
If c2 goes down, then up, the list becomes "c1, c3, c2"
This can be considered harmless for most policies, but impacts OrderedPolicy
Even for other policies like RoundRobin, the sequence c1, then c2, then c3,
then c1 again won't be respected
