The delete/recreate task makes me think it is a bug in outlook.

You should verify wether your client already installed the latest
outlook2003 service pack.

I known that Outlook 2003 with no service pack has problems with some POP3
server that keep it from deleting all the read messages from the server (it
deletes only 2 of them and download the others again and again). Maybe
you're hitting another bug.
