Spool threads are not spawned at every mail arrival.

Spoolmanager at boot instantiates N spool threads and they poll for incoming
messages to process.

So it probably happen when you had N mails (the number of spoolthreads)
matching your "hanging" mailet.

To remove the mail after processing you have to call
mail.setState(Mail.GHOST).
