Please enable "debug" logging for the spoolmanager in your
apps/james/SAR-INF/environment.xml (restart james after this).
In the logs folder you will see a big spoolmanager log file that will
tell something about every matcher/mailet called for each message.
This way you will see what is placing the mails in spam.
If you use the Bayesian mailet, how do you populate the Ham/Spam db? If
something wrong ends up in these db you could have the weird behaviour
you see.

with release 2.3.1.
