Unfortunately, No to both questions.

You could have a MX record in your DNS server per (sub)domain (no need for a hardcoded gateway in that case), but it will still not be per-user.

You need to restart the server in case of fetchmail change.

I fear James is not yet ready for you usecase... but proxy is a frequently asked, so you don't hesitate to add it on the Roadmap for Apache James Server 3.1 [1]
