As Cezar suggested, mentioned servlet before the actual name of the servlet (http://IP:PORT/servlet/SurveyServlet?...)and my html file started talking to the servlet. Warrick's suggestion with regard to removing the <i> markup helped in actually getting my parameters back into my servlet's doGet method. Thanks again to everyone who took time solve this!
