First, your servlets should be in a dedicated servlet folder such as WEB-
INF\classes. Your web pages and such should be at the root of the web app
(organized in whatever fasion you want). So your form and servlet shouldn't be
in the same folder unless your form is generated by a servlet.
Now your form probably needs to have a path that begins at the root so
like "/WEB-INF/classes/servlet_name"
