Except it is a shared JVM and locally it is private.  Surely this will make
some differences.  What differences? and how might they affect my problem?

Will get 4.0.3 though and see what happens.

 Off the top of my head:

 - Download 4.0.3 and try it locally. If it works, you know it's
   your hosting provider, if it doesn't, you know it's you and
   can debug locally.

 - Look at the logs. Turn debugging way up in server.xml. Look
   at the logs again.
