Seems to me that the connection is not getting back (returned) to the pool after the request. Have you used your own implementation of Connection Pool? Maybe you should debug the number of connections in the pool on each request.
BTW there are some great industry-strength implementations of JDBC connection pools but each have their pros and cons. Depends on your Connection pool implementation I believe. If you are using dbcp, it doesn't actually "close" the connection but returns it to the pool on conn.close().
