I was able to solve this actually, but I'm not sure anymore what fixed the specific issue I was having.  I think it was more a problem with my setup in general, so chances are you won't run into the same problem.

SFLphone works fine in an LTSP environment.  The only thing that's very recommendable is to run it directly on the thin client, rather than on the terminal server.  LTSP transfers in/out audio over the network between the server and the client, so SFLphone works, but the transferring of raw audio causes a lot of unneeded network traffic and a slight delay with VoIP telephony.

You can install SFLphone directly on the client, by installing it in the LTSP client chroot (usually /opt/ltsp/i386).  You can execute local commands on the thin client by running "ltsp-localapps <command>" on the terminal server after login.
