First thing I would check is the volume controls for Pulseaudio

As I understand it SFLPhone understands Pulsaudio, which means that there are separate streams for ringtone and the calls.

Load up pavucontrol and leave it open on the desktop in the playback tab

Establish a call with one of th echo test sites (sip:[email protected])

Check the volumes on the playback and recording tabs.

The sound is set to use pulse audio. In the same machine, other clients such as Empathy can establish two way audio conversations without problems.
