The short answer (for now) is that you either need to create an account with a 
VoIP provider (like the one you mention), or take the more complicated route of 
setting up your own VoIP server (i.e. with either Asterisk and Freeswitch) and 
creating accounts on it for you and other peers to use. I would recommend the 
former option.
