Although I can't even come close to Christian's knowledge on Sharity, I do have a
similar setup as you.
I'm using an SGI O2 running IRIX 6.5.8 connecting to a Win2k box.
My O2 is a Samba Server.
I'll give input on your comments below.....

Something here isn't consistant. Why do you have to restart Sharity, if it has been
running fine for 9 months?
Sounds like you have been losing connectivity in the past, and restarting Sharity
would fix it?
It's rare that I ever have to restart Sharity.
What version of Sharity are you using on the IRIX box?
Mine is 2.4 beta 8.
I did have trouble trying to install a later version on IRIX.
Since I don't know how to compile the source, I used a IRIX binary, which never worked.
So, I'm still using 2.4 beta 8 which has been rock-solid.

I can get this error, too if I type in a bogus computer name.
Did you double-check your computer name you're logging in to?
Maybe there's a typo. (Sorry to state the obvious, you probably already did this.)
The first thing I would do is try to ping the Win2k box.
(Sorry, you'll have to open a UNIX shell).
at the command prompt type ping, followed by your Win2ks name that you're trying to
mount. Something like...
ping yourpc
If you don't get success there, try pinging it by IP address, like
ping 192.100.95.10
If you get success there, which I would think you will, try using the IP address in
the Sharity GUI instead of the name.
Another ping you should try is that of your WINS server.
You have the right information, but can you ping that server?
Try pinging it by IP address. If you can't, it won't be of much help to resolve IP
addresses to Netbios names.

You're scaring me here. What is the path you're trying to mount?
You say you're getting permission denied when trying to create a folder on the IRIX
machine as root?
Try this.......
NOT as root, but as your normal login account on IRIX, open a command shell (sorry
again, but just do it. :)
type this, substituting your PCs name, of course.
cifslogin //YOUR-PC
When it prompts for a password, enter it
Then, type this
cd /CIFS/YOUR-PC
see if it lets you there. You should be able to see the shares by typing
ls
