FYI - this is the response I got from the Shuffle Putter guys.
At least they answered.

id (ii) has to do with the shaft angle being more than 20* off
vertical (It looks USGA views the surface that hits the ball as
the face (duh) and not the toe)

4b says that heel to toe distance has be greater than the face
to the back.

Thank you for your interest in the Shuffle Putter. I apologize for the delayed
response to your question regarding our putter. We have received ruling from
USGA regarding the status of Shuffle Putter. At this time it does not conform
with Rules 1d(ii) and 4b in Appendix II.

However, we are currently awaiting their response to our modified design which
has been resubmitted for review. I do not know when we will receive the ruling
on the modified putter.
