Thank you all for the feedback (keep it coming) Actually the original solution was built as a web based search/form engine over MySQL (using PHP), but the configuration to get it to add data to multiple tables (there are 5 of them) has presented challenges to me.
Not sure if it might be worth persuing as I saw Ed R reference $20 for a version he has.
