We have some feature requests open for being able to choose the directory
organization scheme, but nothing that quite matches this.  I'll generate a
ticket for it shortly.

Shotwell already has code to handle the case where two images with the same
filename, but different contents get imported (the second one it encounters
is renamed), so that particular piece of the puzzle may already be solved.
