The problem with this approach is that looking up EXIF dates is
unfortunately quite slow. If we went this route, we'd have users
complaining that import is too slow.

I mean filesystem based comparision, exif can be used to double check. I do not know how import is done in 0.11 version. But 0.10 version do not change filenames after import. I assume all filenames from DCIM still the same, like: IMG0001.NEF, IMG0001.JPG.
