I had the same problem and solved it with a mixture of tricks I found
here on the mailing list. I don't know which of them is really necessary.
- I used 512MB swap (maybe thats not the problem when you say 128 wasn't
even full)
- I stopped frameworkd: "/etc/init.d/frameworkd stop"
- I first did "opkg install opkg libopkg0"
- I upgraded all frameworkd files: "opkg list_upgradable | grep
frameworkd" and then "opkg install <pack1> <pack2> ..."
- I stopped frameworkd again (since it seems it started up again):
"/etc/init.d/frameworkd stop"
- Now I finally did the normal "opkg upgrade"

I found another "solution" in the SHR User Manual
(http://wiki.openmoko.org/wiki/SHR_User_Manual#System_Customizing). It's
a script which installs one upgradable packet after another and spares
out the kernel packets (I don't know why ;)). The problem for me was,
that I couldn't answer the config-file-overwrite-dialogue and the script
always stopped at this point.
