This is not a trivial question :-). In theory both approaches give you
the same result. If the updates are very big, opkg is likely to not
succeed (especially if you are not using a swap file, as it will
download ALL packages to be updated to /tmp which is a tmpfs on the FR,
so some random process are likely to crash etc...). I do try to update
first, and only when things go horribly wrong, I reflash. In any case,
backing up your contacts etc is recommended....

I don't think there was any big change since January in shr-testing
though. I plan to merge in current shr-unstable soon though and give it
a test run as a new shr-t candidate.
