Andrew P confirmed that the question is solved:
I did resolve the problem, but not sure that I found the cause or the
solution. I was running Sikuli on a VMWare ESXi 5.0 machine (Windows
7). Someone recently changed the graphics adapter on the machine
without reinstalling ESXi. Once I reinstalled ESXi, these random
failures disappeared. What's interesting is that the virtual machine
was exactly the same as before (I didn't overwrite the partition just to
see what would happen.)
