I would have to second the homozon suggestion.  My
only comment about it, tho would be to move slowly. 
If you do decide to try it, start with only 1/8 tsp.
in a glass of water and gradually move up.  Usually,
even that will get things moving gently.  Increase the
dosage until you find a comfortable amount that
gets/keeps the bowels moving.  It must be followed
with a glass of water with some lemon/lime (usually
1/4-1/2 lemon or lime will do) juice to activate the
ozone.  It's just such a great product.  It is also a
product that you can stay on forever with only
benefits, imo, and will give you added energy because
of the ozone.

Personally, if apples and such cause a problem for
you, you might also think before going on a fiber
supplement.  Have you ever put some medimusil in a
glass of water and just left it there for awhile?  It
kind of swells up.  The same thing is going on in your
colon.  If, in fact, you do have some kinks in some
places, just that extra bulk might even cause more
problems which could lead to a blocked bowel, a VERY
serious condition.

Even tho your weight is low, I still would consider
doing some king of fasting.  Personally, I like the
lemon cleanse.  That will also thoroughly clean you
out, plus give your body/organs/colon/everything a
rest so that possibly it can recover.  Even if one
does end up with a blocked bowel, the first thing the
doctors do is stop food intake.  The lemon cleanse is
only for 10 day, but you can stop at any time and
whatever weight you loose can easily be put back on.

These are just some suggestions that are known to help
bowel function that you might want to consider. 
Hopefully, you can avoid surgery, but don't limit
healing to any one way.  If you have to have the
surgery, then you have to have it, but I would
certainly try everything I could before that avenue.
