You summed it up for me well. Though it is not conscious, or at least conscious as we define it, I believe people do indeed know "what they're doing." AND it is still hard to watch. It is hard to watch the whole "drama" involving the medical system, the beliefs, the fears, the need for authority, the cultural context, the distress of the person caught in the drama. It is hard to know and not be allowed to give. It is a great lesson and discipline to "let go" ... it requires strength and compassion.
