I believe that a doctor can sometimes see a tumor in the bladder, and
might have a pretty good idea if it is cancerous or not by its
appearance, but for definite proof a biopsy is likely necessary. It is
also possible they might be able to get that information from looking
for and at any cells that end up in the urine, but I have never heard of
that being done.
