Country Health Ltd. (out of Idaho). It's formula #39, called Parasit-Chelation. It contains Diatomaceious Earth and EDTA, and I can personally attest to its successful chelating abilities. The naturopath who formulates it exposes that and all of his other supplements to a radionics machine to give them some more "zap" (this he doesn't advertise). The phone number is 208-397-4156. You have to activate the capsules with lemon juice, by the way, so don't order it if you're allergic to citrus. Also, do NOT under any circumstances spill out the capsule's contents and eat it without the capsule -- diatomaceous earth can "catch" in the throat because of its naturally-occurring silica points. However, it doesn't do any damage once it's in the stomach, and in my case and with other people I know the desired results have been achieved relatively painlessly. Good luck with your healing.
