I am only guessing here, but since you are dealing with an electrotherapy machine, it seems logical that perhaps this "neutral" or "indifferent" secondary terminal reffers to the center one that is normally grounded on a center tapped secondary. From what I understand such a secondary has two sets of windings, each encompassing the changing flux on the transformer core, but each of these windings are made in opposite winding directions, and this is the TRUE connotation of what is termed a set of bifilar windings. Because each winding is oppositely wound, they generate opposite voltages on the endings with respect to ground. This is also how the power Co. delivers household voltage, and why one of the prongs for 120 volt outlets is referred to as the hot one, and the other one is actually tied electrically to ground. If we use both hot sides from the distribution transformer then we have 240 volts from the transformer, so that whenever one AC cycle is making a positive 120 volt peak, the other side is making a negative 120 volt peak, and the difference between these summed voltages becomes 240 volts. High voltage transformers are commonly constructed with center tapped secondaries. I was doing a form of electrotherapy some time ago where a magnet winding with the magnets N top surface in contact with a 60 hz high induction coil resonant rise of voltage, where eventually I conclusively proved by later experimentation that this SrFe ceramic magnet stack would actually "amplify" this high voltage signal. This high voltage signal would go to a small 4 inch neon, through an arc gap, and then to an aluminum foil plate. A person was then treated by placing this foil in contact with his body. The body in question must not be in very close proximity with the ground, such as standing on cement with bare feet ect, or he would experience a shock. In fact the subject sitting on a couch, having shoes on with an intervening carpet between the cement floor of the house, could actually reduce the volume of the neon body discharge by raising his feet off the carpet! Even though this all deals with non conductive substances at some distance to ground, there is still a very real electric field going between the person and that ground. A larger 20 inch neon directly connected to this resonant coil system and end connected to a ground rod will blink at a close value to the resonant frequency of the earth. For the magnet application of neon therapy the subject feels absolutely no shock. However that subject can never touch another person, even though both are ungrounded, or both persons will experience quite a shock. The high frequency emanations coming from that subjects body can be scope measured by having him stick his finger through the opening of a scope monitored coil, which will cause the coil to resonate at the coils own natural resonant frequency. These same type of signals can also be measured coming from the neon bulb itself, but the body signal is twice as strong. The body, like the magnet has the effect of amplifying these signals.
I was doing these things as a version of Rife Neon Therapy which can be found at http://www.rifetechnology.com/intro.htm I do not think all of this is a bunch of huey, as I was able to do a pepper seed germination experiment using these effects, and did find a pronounced difference in germination rates. I also used this same coil system in an entirely different set up to produce colloidal silver water, but later on I found that a variac- NST -low voltage diode application could produce the same current limited regimen, and I also was able to produce it in a way so that it stays clear after production, with a 4 fold increase of conductivity, while keeping the current levels under 1 ma, with ~ 1 inch separation of square silver electrodes commonly available at coin shops. I market this for 8 dollars/16 oz, not including shipping costs.
