I don't bother with the public IP in my client hosts as I never 
originate mail on my mail server.

To the best of my knowledge there is no way to tailor the message 
based on which entry in the black listed triggered the rejection.

You need to put the 127.0.0.X numbers you wish to block into the 
black listed hosts lists. The mechanism allows you to use a RBL that 
returns different codes for different violations. You can then select 
which ones you care about by putting the appropriate codes into you 
black listed hosts.

I don't know how to modify the response message for this rejection.

No. SIMS only processes the "envelope" of the mail. The Received: 
lines are in the body of the mail. I have heard that Communigator can 
do this.

All SIMS rules are server wide :) But the specific item you are 
asking about is a Communigator only feature.
