There are 2 ways I know of:

1) Send mail from an account outside of your client list to someone
outside your client list. Moderately difficult because you need an
"outside IP" and you need to use a relaying address structure
(described somewhere in the SIMS docs).

2) Have someone telnet into your server from outside and see if it
accepts relay mail. In the OS X terminal window it would look like
this:

>telnet clientfromhell.com 25
>Trying 209.xxx.xxx.xxx...
>Connected to clientfromhell.com.
>Escape character is '^]'.
>220-Stalker Internet Mail Server V.1.8b9d11 is ready.

>MAIL FROM: [EMAIL PROTECTED]
>250 [EMAIL PROTECTED] sender accepted
>RCPT TO: [EMAIL PROTECTED]
>571 [EMAIL PROTECTED] we do not relay.
>quit
>221 xxxxxx.com closing connection
>Connection closed by foreign host.
