Here's the hack, in outline form:

Every address gets two accounts set up for it.  SIMS only delivers to 
one of these,  in folder mode. Some Other Process (perhaps a script 
attached to the folder) takes each file delivered to that folder and 
plays with it however it pleases: add lines, change words, swap the 
word "SPAM" for any noted marketing terminology, look for viruses, 
etc. This process concludes by placing the corrupted^Wenhanced 
message onto the end of the mailbox file for the second account (the 
one that SIMS never delivers to) which is the one the user picks up 
from.

Oh, you say you want this on *OUTBOUND* mail too???  Ouch, that's 
harder.  Outbound messages are not let go by SIMS except for a 
fleeting moment which you are likely to be incapable of catching with 
some Other Process unless that Other Process is running at interrupt 
time, in which case you can't DO anything useful in this case, 
because 'useful' would involve snatching that file out of the queue. 
Outbound mail fiddling would demand mods to SIMS.
