My guess is that I'm finally hitting a wall there with SIMS (or rather OS 9). It can handle the load ok, until someone goes off line then my server grinds to a near halt and gets unstable as the queue directory gets in the 20k+ number of messages...

I would seriously consider not supplying or using secondary services. Most sending MTAs will retry for a day or so - let them handle the load. Depending on your ISP or DNS service, you may be able to use an offsite server as a secondary. In my case, I use the secondary services at EasyDNS. However, virtually all of the spam I get is sent directly to the secondaries - thus bypassing my SIMS filters.
