Does that mean that such proxies have to be registars for all the users they serve
i.e., those inside the LAN?
If not there should be a registrar which all users inside the LAN are registered to.
In that case how will the proxy communicate with the registrar?

For outbound requests, the outbound proxy does not need registrar functions.
It routes based on DNS. For inbound requests, its not an outbound proxy
anymore, its the proxy/registrar for the user. Thus, the statement that "the
outbound proxy needs to keep a database for the users inside the LAN" makes
no sense.
