This is an issue for your stack interface. When the application requests
reliable provisional responses, it can do so either in a "priority" way,
which means that these should definitely be sent, even if the call is
answered, or in a "non-priority way", which means a call answer should
discard the unsent reliable provisionals. The app can decide which case is
needed. The behavior in either case for the stack is clear. You send any
priority provisionals before sending the 200.
